Minors Younger than 18 years old traveling to or from Botswana, must complete the document in accordance with the following provisions.


I. Traveling with both parents
- Passenger must hold an original full birth certificate containing information about the parents, or a certified copy, or an equivalent official document containing details of the minor an the minor's parents.

II. Traveling with 1 parent
- Passenger must hold an original full birth certificate containing information about the parents, or a certified copy, or an equivalent official document, containing details of the minor an the minor's parents, and
- A certified copy of the other's parent's passport or ID card, and
- A parental consent affidavit from the absent parent issued by a Commissioner of Oaths
- In case the other parent is deceased, a death certificate required.

III. With adult person, other than a parent
- Passenger must hold an original full birth certificate containing information about the parents, or a certified copy, or an equivalent official document containing details of the minor and the minor's parents, and
- A certified copy of both parents passport or ID card, and
- A parental consent affidavit from both parents issued by a Commissioner of Oaths.

IV. Unaccompanied
- Passenger must hold an original full birth certificate containing information about the parents, or a certified copy, or an equivalent official document containing details of the minor and the minor's parents, and
- A certified copy of both parents passport or ID card, and
- A parental consent affidavit from both parents issued by a Commissioner of Oaths.

All document provided by passengers must be originals or certified copies and must be in English. The parental consent affidavit must be issued within 6 months prior to the first date of travel.
